Consent to Treat

I hereby voluntarily consent to evaluation and treatment by a licensed Physical Therapist at this event. I understand that the treatment may include, but is not limited to, physical assessment, therapeutic exercise, stretching, soft tissue techniques, and other physical therapy interventions deemed appropriate.

I acknowledge that physical therapy involves inherent risks, including but not limited to muscle soreness, fatigue, or aggravation of pre-existing conditions. I have disclosed all relevant medical conditions, injuries, and medications to the best of my knowledge. I understand that I may refuse or discontinue treatment at any time.

I authorize the Physical Therapist to provide care within their scope of practice under Arizona law and release the provider and associated entities from liability except in cases of gross negligence or willful misconduct.
